Student Activity Form

The Student Activity Form should be completed and returned to the student's guidance counselor in order to help the counselor complete recommendations.

Right - Click on the image to the left and choose "Save Target As..."
Then either save it to your home directory (school) or your computer (home).

The document is in Microsoft Word (doc) format, so you should be working from a computer with Microsoft Word installed.

When you complete the form, save it and print a copy for your guidance counselor.
