Sports Physical Form
Download this form. Bring it to your doctor when you get your sports physical. Mail or bring the completed form to Mrs. Cristadoro, PHS nurse. An up to date physical form must be submitted to the school nurse prior to the first day of practice or try-outs. (By August 15 for Fall Sports.)

Sports Permission Form
Download this form. Complete it. Sign it. Have your parents sign it. Send this form to the Athletics Office, Pomperaug High School, 234 Judd Rd., Southbury, CT. 06488.. Send this form by August 15 for Fall Sports, by November 15 for Winter Sports, by March 15 for Spring Sports.)

These forms need to be completed and submitted to the athletics office with a check for $100. This is required prior to the first day of practice or try-outs. (By August 15 for Fall Sports.)

School Spirit Wear Order Forms
All Spirit Wear can be Ordered at Any Time of the Year. Lettermen Jackets must be Ordered no later than November 1st, to be in by Christmas. Sweatshirts, Sport Bags, and Supplex Jackets can be ordered by Dec 5Th to be in by Christmas.
All forms are available in the Athletic Department Office by the New Gym.
